What to Expect
In most cases, you will have an initial consultation before any procedures maybe scheduled. During this visit, Dr. Cadoff will review your previous medical records, test results and/or X-rays. Additional tests may be ordered if needed. If you require a procedure or surgery, he will explain the procedure to you and answer any questions you may have. You will be provided with pre-surgery instructions, and our friendly staff will assist you in scheduling your procedure.
Prior to your surgery, Dr. Cadoff will stop in and answer any last-minute questions you may have. He will also check on you after your surgery. A follow-up visit may be scheduled.
Once you are discharged from the hospital, you will be under your primary physician’s care. However, our involvement doesn’t end there — our office will call to see how you are doing and act as a liaison between your doctor and the hospital.
Preparation for Your Appointment
- Government-issued Photo ID.
- List of all medications you are currently taking (names and doses). Many patients find it easier to simply bring their medication bottles with them.
- Insurance card(s).
- A parent or legal guardian if the patient is under 18. A parent or legal guardian must be present during the entire visit.
- Records from other health care providers, if applicable.
- Completed forms including New Patient Questionnaire and Registration Form (see below for forms).
